Hawthorne gaat over Christina Hawthorne (Jada Pinkett Smith). Zij is hoofd medisch adviseur aan het Richmond Trinity Hospital. Dr. Tom Wakefield (Michael Vartan) werkt als hoofd chirurgie bij hetzelfde ziekenhuis. Collega zusters zijn o.a. Bobbie Jackson (Suleka Mathew), Ray Stein (David Julian Hirsh) en Candy Sullivan (Christina Moore). Camille (Hannah Hodson) is Christina's dochter uit een multiculturele relatie en Amanda (Joanna Cassidy) is Christina’s schoonmoeder, welke toevallig ook een lid is van het bestuur van het ziekenhuis.

The Starting Line
